How to obtain a copy of a Czech Birth Certificate
You are able to obtain a copy of a Czech birth certificate for someone born in the Czech Republic through the Honorary Consulate of the Czech Republic in Malta.
The application form for a duplicate birth certificate needs to be filled in with boxed 1-6 on the first page and boxes 23 to 32 on the second page. The document should however not be signed.
Make an appointment with the Honorary Consul by emailing info@czechconsulatemalta.org suggesting 3 days and times two as to enable the Consulate to find an appropriate time.
Bring your identification document with you (a Czech Passport or Czech ID Card). The form will then need to be signed in the presence of the Honorary Consul. Your documents will be sent to the Embassy in Rome who will forward them to the appropriate authorities in Rome.
You will need to specify whether you just want the original birth certificate in Czech or have the translated and/or aposteeled version of the birth certificate dependant on your intended us of the Birth Certificate. Costs will vary according to the exchange rate tariff of that month and the version of the birth certificate required.
