What if you have lost your Czech Passport or Identity Card?

  • BACKGROUND

    If you are a Czech citizen and you have lost your Czech passport or had it stolen, it is important for you to report this in person to any Police Station in Malta or Gozo. You will be required to provide proof of a Police Report to replace your documents and to obtain an Emergency Passport should you require one.

    If you have lost your passport, or had it stolen, you are still able to travel between Malta and the Czech Republic using your Czech identity card, assuming that you still have this.

    If you do not have either your passport or your Czech national Identity Card, you are able to apply for an Emergency Czech Passport via the Honorary Consulate for the Czech Republic in Malta. following the procedure alongside.

    This Emergency Passport can be used to travel only back to the Czech Republic for one journey where you will be able to apply for a replacement full passport or national identity card. You cannot obtain a full passport through the Honorary Consulate.

    The Honorary Consulate cannot issue replacement identity cards.

    IN CASE THAT YOU URGENTLY REQUIRE A REPLACEMENT TRAVEL DOCUMENT BUT THE HONORARY CONSULATE IS CLOSED, please contact the 24 hour Emergency phone number of the Embassy in Rome on +39 335 310 450. PLEASE ONLY USE THIS NUMBER IN AN EMERGENCY.

  • PROCEDURE

    If you have lost your Czech national ID Card and/or passport, you have to report their loss to the police in Malta. This can be done at any police station or using the Malta Police On-Line Reporting system.

    You are also legally obliged to report the loss of your ID Card to the Czech Government.

    Download the Notification of Loss of Czech ID Card form, filling it in and send it to the Honorary Consulate on info@czechconsulatemalta.org

    If you have your passport to travel with then use that and, after having reported the loss of your ID card, on your return to Czechia, follow the procedure of obtaining a replacement ID Card. Replacement cards cannot be issued outside of Czechia.

    If however you do not have your passport to use for travelling, then you have to apply for an emergency passport. Please follow these instructions outlined in the FAQ Applying for an Emergency Travel Passport.
