How do I obtain an emergency travel passport for an infant child?

  • What's the Problem?

    This FAQ defines the process to obtain an emergency travel document for an infant child to allow you travel to Czechia to register the infant and so as to complete the registration process and to obtain a passport in their name.

    Please note that an Emergency Passportcan only be used for a trip to the Czech Republic ad it will not be able t be used to earn tanother destination thereafter. You must therefore make arrangements wile in Cechiato obtain a standard passport for the infant to be able to travel back.

  • The Procedure

    You will be able to obtain an emergency travel passport for an infant which will enable you to travel to Czechia to complete the registration processes for your child and obtain a full passport.

    The Emergency Travel document will only allow you to travel to Czechia, although transit stops are allowed, and can only be used once. You will need them to ave obtained a full passport for their return journey.

    You will need to undertake the following steps:

    1. Fill in the Emergency Passport application form. This can be filled on-line without printing and can be saved as a PDF. Do not sign the form as this needs to be signed physically in the presence of the Honorary Consul.

    2. Scan and send a copy of the Birth Certificate issued in multi-lingal format by Identtà (Identity Malta).

    3. Scan a copy of the parent(s) Czech Passport or Identity Card.

    4. Send these 3 documents to the Honorary Consulate on malta@honorary.mzv.cz. These will then be forwarded by the Honorary Consulate to the Embassy in Rome for verification and authorisation to issue the passport.

    5. Wait for an appointment to visit the Honorary Consulate

    6. Bring to your appointment your original passport, the multi-lingual birth certificate, two passport sized photos of the infant and the filled but unsigned application form.

    7. A fee is applicable as per the published rate issued by the Czech Ministry of Foreign Affairs which currently is EUR 16.00.
