What if you have lost your Czech Passport or Identity Card?

  • BACKGROUND

    If you are a Czech citizen and you have lost your Czech passport or had it stolen, it is important for you to report this in person to any Police Station in Malta or Gozo. You will be required to provide proof of a Police Report to replace your documents and to obtain an Emergency Passport should you require one.

    If you have lost your passport, or had it stolen, you are still able to travel between Malta and the Czech Republic using your Czech identity card, assuming that you still have this.

    If you do not have either your passport or your Czech national Identity Card, you are able to apply for an Emergency Czech Passport via the Honorary Consulate for the Czech Republic in Malta. following the procedure alongside.

    This Emergency Passport can be used to travel only back to the Czech Republic for one journey where you will be able to apply for a replacement full passport or national identity card. You cannot obtain a full passport through the Honorary Consulate.

    The Honorary Consulate cannot issue replacement identity cards.

    IN CASE THAT YOU URGENTLY REQUIRE A REPLACEMENT TRAVEL DOCUMENT BUT THE HONORARY CONSULATE IS CLOSED, please contact the 24 hour Emergency phone number of the Embassy in Rome on +39 335 310 450. PLEASE ONLY USE THIS NUMBER IN AN EMERGENCY.

  • PROCEDURE

    To apply for an emergency passport, follow these instructions:

    1. Report the lost or stolen passport or identity card to any police station in Malta or Gozo. The police will provide you with a card with the police report number.

    2. Send an email to the Consulate on malta@honorary.mzv.cz reporting the loss providing the following information:

    a. First name

    b. Surname

    c. Email address to which you have access while in Malta

    d. Phone number in use while in Malta

    e. Date of Birth

    f. Passport Number

    g. Czech Identity Card Number

    h. Date of loss of passport

    i. Photo/scan of your passport and/or identity card number

    3. Download the Emergency Passport application form on which you are able to fill in the fields directly on the form. It is not required to print the form. DO NOT SIGN OR DATE THE FORM, as the signature must be affected in the presence of the Honorary Consul.

    4. Send the filled in form as well as the photo/scan of the passport and/or identity card to malta@honorary.mzv.cz. The Honorary Consulate will pass on the form and the copies to the Czech Embassy in Rome for verification and authorization for the issuing of the emergency passport.

    5. Obtain two passport-sized colour photos of yourself.

    6. Do not come to the Consulate until the authorization has been received by the Honorary and communicated to you and you are contacted by the Consulate on the email or phone number provided by you and given an appointment to come to the Consulate.

    7. Once you have received your appointment, come to the Honorary Consulate on the indicated date and time, bringing with you all required documents, forms and photos. See location.

    8. The applicable fee is currently € 17.00 but may vary from time to time due to exchange rate fluctuations.
