How do I register the birth of a child?

Application for a Czech birth certificate and a certificate of Czech citizenship

Please Note: Registrations of children through the Honorary Consulate of the Czech Republic in Malta have been suspended until mid-September due to a limited availability of staff at the Czech Embassy in Rome. Apologies for this situation.

This information is applicable if your child was born in Malta from at least one parent who is a Czech citizen and you wish to apply for Czech citizenship and a Czech birth certificate for your child.

If your child was born in Malta, you must apply for his or her first Czech birth certificate. If the application is being made after the child is 1 year old then an application for a certificate of Czech citizenship is also required. These documents are necessary in order for the child to obtain a Czech passport.

Following the availability of all the required documents, shown below, an appointment then needs to be made with the Honorary Consul in Malta or the Embassy in Rome and all the requested documents, unsigned by the presenting parent(s), must be submitted by email when requesting an appointment.

PLEASE DO NOT COME TO THE HONORARY CONSULATE WITHOUT HAVING FIRST SUBMITTED THE REQUIRED DOCUMENTS VIA EMAIL AND MAKING AN APPOINTMENT AS THE APPLICATION WILL NOT BE ABLE TO BE PROCESSED.

You can apply via the Honorary Consulate in Malta or in person at the Czech Embassy in Rome after having made the appointment. The Honorary Consulate in Malta or the Czech Embassy in Rome will then forward the application to the relevant authorities in the Czech Republic, which are the Prague 1 Municipal Office (for the issue of the citizenship certificate) and the Special Registry Office in Brno (for the issue of the birth certificate).

Please note that it takes approximately 3-6 months to process the application.

The parent or parents attending the appointment should not sign or date the documents before they are presented in person to the Honorary Consuland signed in person after verification.

  • If both parents of the child are Czech, only one of them needs to submit the application and required forms:

    • their own birth certificate AND

    • their proof of Czech citizenship and that of the other parent AND

    • a signed agreement on the surname in the "birth registration" form. The parent or parents attending the appointment at the Czech Honorary Consulate in Malta should not sign the documents before they are presented in person to the Honorary Consul.

  • If one of the parents is Czech and the other is not, the application is to be submitted by the Czech parent with:

    • a copy of the identity document of the other foreign parent AND

    • a signed agreement on the surname in the "birth registration" form.

    • The foreign parent does not submit his/her birth certificate.

If the child was born out of wedlock, the personal presence of both parents is required.

The following completed forms and documents must be submitted either to the Honorary Consulate in Malta or the Embassy to apply for a certificate of Czech citizenship and a birth certificate for a child born in Malta:

  1. Application for a Certificate of Czech Citizenship - only required if child is older than 1 year.

  2. Questionnaire to establish Czech citizenship;

  3. Proof of Czech citizenship of the parent(s) - for instance a valid Czech passport, a Czech ID card or certificate of Czech citizenship (note: the foreign parent submits only a copy of the identity document);

  4. Application for birth registration;

  5. The child's birth certificate issued by Identità in Malta in the Czech language. You can apply directly to Identità in Malta for a birth certificate with the so-called multilingual standard form in accordance with EU Regulation 1191/2016 via this link. (The multilingual form replaces the official translation into Czech). If the translation is not made by a Czech court translator, the embassy must verify it. If the child was born in a non-EU country, the birth certificate must also be certified with a higher certification (super legalisation or apostille clause). Information on certification is available upon request from the consular section.

  6. Czech birth certificates of parents (note: the foreign parent does not submit);

  7. Czech marriage certificate of parents;

  8. If the parents were not married at the time of the child's birth and are both listed on the birth certificate, they shall submit:

    • A foreign document of paternity recognition (e.g. an extract from the birth register with an official translation into Czech. Exceptionally, it can be replaced by a Consent Declaration of Establishment of Paternity;

    • A statement of what surname they wish the child to use (there is no form for this statement);

  9. If the child's mother is not a Czech citizen and the child's father is listed on the foreign birth certificate:

    • If the child's mother is a citizen of an EU Member State, Switzerland or a state that is a party to the Agreement on the European Economic Area - a foreign document of recognition of paternity with an official translation into the Czech language may exceptionally be substituted by a Consent Declaration of Establishment of Paternity ;

    • Duly translated Confirmation of paternity by genetic testing in an accredited laboratory, if the mother is not an EU citizen .

If you wish to register your daughter in the register of births without the "-ova" ending, you must also fill in the form Application for registration of surname in the register of births in the male form. You can consult with us when you submit your application for a child's surname.

A fee is payable in cash at the time of application in EUR according to the current tariff published every month by the Czech Ministry of Foreign Affairs.
