How to obtain an extract from your Citizen’s Data.

Czech citizens getting married overseas, including in Malta, would be request to provide a Czech marital status certificate to confirm their single status. This certificate can be obtained through the Honorary Consulate of the Czech Republic in Malta but the process takes time and we would strongly recommend that you start this process as soon as possible.

  1. Citizens are required to fill BUT NOT SIGN this application form for the Request of Citizen Data.

  2. Scan a clear copy of the filled in but as yet unsigned form

  3. Take and print a copy of your passport or Czech identity card.

  4. Write on the printout the following text: “souhas s pořízením fotokopie”. This means that you consent to the taking of a copy of your passport or identity document and relates to data protection.

  5. Send the unsigned form together with a copy of your passport or identity card with the copying consent signed to the Honorary Consulate by email. In this email, please also identify the local office of your Czech place permanent residence (Prague, Brno etc).

  6. Once all this documentation has been received, we will pass on this information to the Czech Embassy in Rome to confirm their authorisation to process the document.

  7. Once we receive the authorisation, we will provide you with an appointment to come and sign the original documents at the Honorary Consulate. We will then send all documents to the Embassy. Please bring the form WITHOUT YOUR SIGNATURE, a copy of your Czech passport or Czech Identity Card and fee of EUR 32.00 in cash.

Please note that this process takes approximately 12 weeks.
